The Opportunity
- Pay Rate: $35.00/hour
- Role Type: Temp-to-Perm (Potential for permanent hire based on performance)
- Schedule: Monday – Friday | 8:00 AM – 5:00 PM (In-Office)
- Location: Harrisburg, PA
Primary Responsibilities
- Clinical Gatekeeper: Evaluate all incoming patient referrals. Review hospital records, homebound status, and clinical background to determine home health eligibility.
- Intake Coordination: Gathers essential data and coordinates with clinical managers to schedule admission visits promptly and accurately.
- Insurance & Compliance: Verify insurance coverage and ensure "face-to-face" encounter documentation is completed before care begins to ensure full regulatory compliance.
- Healthcare Liaison: Act as a trusted partner to hospital discharge planners and case managers. Educate referral sources on agency services to ensure safe and seamless patient transitions.

Qualifications
- Licensure: Active and valid Pennsylvania Licensed Practical Nurse (LPN) license is required.
- Experience: Minimum of one (1) year of healthcare experience. Previous experience in home care, community health, or intake coordination is highly preferred.
- Skills: Ability to analyze complex clinical data, multi-task in a fast-paced environment, and communicate effectively with diverse populations.
- Certifications: Current CPR certification and a valid driver's license with reliable, insured transportation.
